Dental Health Arlington proudly offers affordable dental care to low-income, Tarrant County residents.
At Dental Health Arlington, we understand that dental care is expensive, but we believe that good dental health is key to leading a happy and successful life. Without good dental health, essential parts of every day life, like eating, being employed, and smiling can be difficult. It is our mission to make a trip to the dentist affordable and worthwhile for patients by providing preventive and pain relieving services to low-income residents and to educate children about good oral hygiene habits.
Here Are Some of The Important Updates
All patients returning to our clinic on or after January 1, 2017, are required to comply with our updated Office Policy. We ask that our patients review this document to understand how these updates will effect their future visits.
STEP 1: REVIEW POLICY
Patients returning to our clinic after January 1, 2017, must review and sign our revised Clinic Policy
STEP 2: RESERVATION FEE
ALL patients must pay a “Reservation Fee” of $25 to schedule any appointment. This is simply a deposit to reserve your appointment time. and will be applied to your examination fees
STEP 3: DOCUMENTS TO BRING
When you appear for your Qualification Interview, please bring the following:

Driver’s License or State ID

Proof of residency in Tarrant County

Proof of income
STEP 4: UPDATE YOUR INFO
Please advise our staff if you have had any change in income, address, or phone number..
Accepted Documents:
Proof of Income
Since our services are provided to persons with “low-income,” as defined by the Department of Housing and Urban Development (HUD), you will be required to establish that you meet HUD’s definition of “low-income. Examples include:

pay check stubs (4 payments)

award letter of government assistance
-
letter from someone who is helping pay your bills
Proof of Tarrant County Residency
Since our services are provided to Tarrant County residents only, you will be required to provide proof of your residence in Tarrant County. Examples include:

Utility bill or other official mail sent to you at your Tarrant County address
Identification
Picture Identification from any government authority. Examples include:

Driver’s License
-
Military ID
-
Passport
What to Expect
YOUR NEXT VISIT
Most of our forms have changed effective January 1, 2017. Every patient has to fill out new forms every new year in our clinic, including policy changes, Community Development Block Grant self certification forms and medical history.
PAYMENT
Reservation fees are due at the time of making an appointment. Any balance due on your account must be paid in full one week in advance of your scheduled appointment date. Accepted payment methods include cash, money order, Visa, and MasterCard. We do not accept personal checks.

We provide a variety of low cost dental services on a regular basis however, less routine services are provided subject to availability. Below is a list of some of the services we offer. Fees in both categories are subject to change without notice.
